The intake form on this page is the first step in beginning a grievance with United Faculty. Once you submit, your campus Grievance Rep will reach out to you based on the information you provide. Read the sections below before filing so you know what to prepare and what to expect.

Why file — and what happens next
Every full-time faculty member at Cypress College, Fullerton College, and North Orange Continuing Education is covered by the Collective Bargaining Agreement that United Faculty negotiates and enforces. When contract rights are challenged, the intake form is the first step toward getting the situation reviewed — regardless of your membership status.
If you are a member of United Faculty, filing gets you full representation through every stage of the grievance process: initial fact-finding, formal grievance filing, and any appeal or arbitration that follows. Your Grievance Rep will reach out based on the information you provide, review the situation with you, and walk through the options together.
If you are not currently a member, filing still connects you with a Grievance Rep who will provide initial guidance about your rights under the CBA, help you understand whether the situation is grievable, and explain what filing would look like. Initial consultation is available to any full-time faculty member in the bargaining unit; full representation through the formal stages is available to active members. Many faculty who reach out at this stage choose to enroll during that first conversation, and the Membership Chair can complete enrollment on the same call.
In every case, prompt contact protects your options. The contractual filing window is forty days from the point at which a faculty member should reasonably be aware of the issue, and that clock runs whether or not a grievance is ultimately filed. Reaching out early — even if you are still deciding what to do — is how the forty-day window is preserved.
Information to gather before you file
Most of what the intake form asks for is standard, but a few fields require details you may need to look up before submitting. Having these ready when you start the form will make the process faster and gives your Grievance Rep what they need to act quickly on your behalf.
Prepare the following before you begin:
- Your contact information — full name, campus (Cypress College, Fullerton College, or North Orange Continuing Education), division or department, preferred email, best phone number, and the times you are most reachable.
- Your supervisor's name and contact information — the dean, department chair, program manager, or other supervisor whose actions or decisions are at issue. Include their email and their campus or office if you know it.
- Date of the violation — the specific date on which the incident, decision, or action occurred. If it is an ongoing situation, note the date it began and the most recent date it continued. The forty-day filing window runs from the point at which you should reasonably be aware of the issue, so having this date documented matters.
- A statement explaining the grievance — a written account of what happened, in your own words. Focus on facts (who, what, when, where) rather than characterizations. If you can identify the contract article, policy, or procedure you believe was violated, note it — but do not worry if you cannot. Your Grievance Rep will help identify the applicable article.
- Supporting documents — anything that corroborates your account. Common examples include emails to and from your supervisor, meeting notes, schedules, evaluations, formal notices, class assignment letters, and copies of the policies or CBA sections referenced. You do not need to have every possible document — bring what you have. Additional documentation can be provided later.
What happens after you submit
Once your form is submitted, your campus Grievance Rep will reach out based on the information you provided. Depending on the specifics of your situation, they may ask follow-up questions, request additional documentation, or schedule a time to speak with you directly. Every case is tracked in United Faculty's secure GrievTrac case-management system so that deadlines, contract references, and communications are documented from the first conversation forward.
